Metabolic Fix Trial: Waiver & Release
By taking part in this trial, You accept the following:
I hereby acknowledge that I am voluntarily participating in this consumer trial conducted by Artah Nutrition Ltd (the "Trial") and agree to the following terms and conditions:
Confirmation: I confirm that the information I have provided is accurate and I have disclosed medical conditions/medications.
Assumption of Risk: I am aware that participating in the Trial involves certain risks and hazards.
Release of Liability: In consideration of being allowed to participate in the Trial, I hereby release and discharge Artah Nutrition Ltd, its officers, directors, employees, agents, and representatives from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or in equity, known or unknown, arising out of or in connection with my participation in the Trial.
Photographic Release: I grant Artah Nutrition Ltd the right to take use photographs and/or video recordings of me submitted during the Trial and to use these materials for promotional or educational purposes.
Compliance with Rules: I agree to comply with all rules, instructions, and guidelines provided by Artah Nutrition Ltd during the Trial.
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY FORM AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING MY RIGHT TO SUE. I ACKNOWLEDGE THAT I AM SIGNING THIS DOCUMENT FREELY AND VOLUNTARILY, WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE.
