WAIVER AND RELEASE OF LIABILITY

This Waiver and Release of Liability (the “Agreement”) is entered into between the undersigned participant (the “Participant”) and ARTAH NUTRITION LTD (“Company”) in connection with the voluntary participation in a consumer trial for the product Advanced Multi-Nutrient (the “Product”).

1. Acknowledgment of Participation

By opting into this Agreement, the Participant voluntarily agrees to take part in the consumer trial for the Product. The Participant acknowledges that participation is entirely voluntary and that they have not been coerced or misled into participating.

2. Representation and Warranties

The Participant represents and warrants that:

• They are at least 18 years of age.
• They have consulted with a healthcare provider or determined on their own that they have no known medical conditions that would prevent them from safely consuming the Product.
• They understand that the Product is a food supplement and is not intended to diagnose, treat, cure, or prevent any disease.
• They will follow all instructions provided by the Company regarding the use of the Product.

3. Assumption of Risk

The Participant acknowledges and understands that:

• The Product is provided for evaluation purposes only, and individual reactions may vary.
• There may be risks associated with consuming the Product, including but not limited to allergic reactions, digestive discomfort, or other unforeseen effects.
• The Participant assumes full responsibility for any consequences resulting from their use of the Product.

4. Release of Liability

To the fullest extent permitted by law, the Participant hereby releases, waives, and discharges the Company, its affiliates, employees, directors, officers, agents, and representatives from any and all claims, liabilities, damages, or causes of action, known or unknown, arising out of or in connection with their participation in the consumer trial, including but not limited to personal injury, illness, or adverse reactions resulting from the Product.

5. Indemnification

The Participant agrees to indemnify and hold harmless the Company and its representatives from any claims or liabilities arising from their participation in the trial.

6. Confidentiality

The Participant agrees not to disclose any proprietary information about the Product or the consumer trial process to third parties without the prior written consent of the Company.

7. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of England and Wales , and any disputes shall be resolved in the courts of England and Wales

8. Acknowledgment and Signature

By submitting this form/opting YES, the Participant acknowledges that they have read and fully understand this Agreement, accept the terms voluntarily should they be accepted into the trial, and assume all risks associated with participation in the consumer trial.
